Calm Harbor Adult Care Worker

The Adult Care Worker is responsible for implementing the post-crisis case plan objectives and strategies for each Calm Harbor resident in a manner consistent with standards of care established by State and Federal regulations and the policies and procedures of Elevate, Inc..  This is a pool position to provide extra coverage for nights, weekends and vacations.

Opioid Diversion (TAD) Case Manager

The TAD Case Manager is responsible for overseeing a caseload of a maximum of 20 clients with a primary Opioid Use Disorder engaged in this diversion program, including meeting the goals of reducing recidivism rates and reducing prison and jail populations by diverting non-violent offenders to community-based interventions. This position will assist clients in entering and sustaining recovery, coordinate with treatment providers, and provide direct support to clients throughout their involvement in the program.

Teen Intervene Program Coordinator

The Teen Intervention Program (TIP) Coordinator is a vital member of the Youth Intervention Program team, and concentrates their efforts on improving outcomes for at-risk youth.  The TIP Coordinator targets youth early in their trajectory of substance use, truancy, and/or delinquency, and provides evidence-based early intervention support to help get them back-on-track toward reaching their full potential.
